17. Unrestricted (Or reciprocal) use of chemical and biological weapons - V1: Targeting military formations and sites is the MO. - V2: If friendly civilians harmed, or it is authorized by high command/is reciprocal, target civilian sites for revenge attacks.
16. Unrestricted Targeting of civilian and military infrastructure - V1: Soldiers may be executed unless ROE or agreement dictates otherwise - V2: If no agreement exists or an existing one is broken, assume V1.
All-Out War Threshold - DEFCON 1
15. Unrestricted attacks against war critical infrastructure - V1: Can include civilian infrastructure if it has a significant military use.

The Escalation Ladder is more of a guiding hand rather than 'THIS IS THE NEXT STEP', and while it is generally good to follow the guidelines and to work in spirit of them situations may require unique approaches that roughly correspond to the REL/MRL (But are not defined within it), or may require 'Leaping' escalation in response to a situation as the circumstances permit.
